By signing this mandate form, you authorize (A) Silvey Fleet SAS to send instructions to your bank to debit your account, and (B) your bank to debit your account in accordance with Silvey Fleet SAS's instructions.

You have the right to a refund from your bank under the conditions described in the agreement you have signed with it. Any refund request must be submitted within 8 weeks of the date your account was debited.
